Informed Consent & Liability Waiver

Massage therapy is provided for relaxation, stress reduction, relief of muscular tension, and general wellness purposes only.

Massage therapy is not a substitute for medical examination, diagnosis, or treatment. Massage therapists do not diagnose conditions, prescribe medications, or perform spinal manipulation.

Clients agree to disclose any medical conditions, injuries, surgeries, medications, allergies, or pregnancy that may affect sessions and to update this information as needed.

Massage therapy involves manipulation of soft tissues and may result in temporary soreness, bruising, or discomfort.

Clients may stop or modify a session at any time.

Any inappropriate or sexual behavior will result in immediate termination of the session with full payment due.

Clients release Alivio Massage Therapy and its therapists from liability for injury or damages arising from services, except in cases of gross negligence or willful misconduct under Colorado law.

Services are provided by therapists licensed through the Colorado Office of Massage Therapy Licensure (DORA).

By booking or receiving services, clients acknowledge understanding and agreement to this consent and waiver.
